Forms for Service Transfers and Purchases
LSERS is pleased to announce the release of our new Form 9, Application for Transfer or Reciprocal Recognition of Service, and Form 9A, Application for Purchase of Service which are located on the Forms page on LSERS website. The Form 9 replaces the Forms 9R and 9T and Form 9A replaces 9IO, 9M, 9RS, and 9SB.
Here are highlights of significant improvements/changes that occurred with the creation of these forms:
- Consolidated multiple forms for similar processes into two simple forms
- Instruction page providing explanation of each section with step-by-step details on how to complete
We hope you appreciate these new forms. Our goal is to streamline processing, reduce paperwork, and simplify the information provided to LSERS.
Our staff is here to serve our members and employers.

Office Closures due to Holidays
Our office will be closed the following dates in observance of the upcoming holidays: Thursday, November 26 for Thanksgiving, Friday, November 27 for Acadian Day, Thursday and Friday, December 24 – 25 for Christmas; and Thursday and Friday, December 31 - January 1 for New Year.
