Updated Form:  Authorization for Direct Deposit – Form 8

The Authorization for Direct Deposit form was updated in September 2019 and is located at www.lsers.net > Resources > Forms; please discard any copies of earlier versions of this form.
Note that the member’s signature line is located at the bottom of the form.  Forms submitted without the signature and date will not be processed. Please assist your retirees or retirement applicants to complete the form appropriately.

Timely Submission of Applications

Timely submission of all applications is extremely important.  This includes but is not limited to retirement, survivor, disability, and refunds.  The receipt date is a critical component of these processes and we want to ensure each member is timely served.  Please assist your employees to avoid delays in submitting any application, as well as, any corroborating documents.

Retiree Return to Work 1006
Minimizing Earnings Reporting Errors

Retirees returning to work under the provisions of La. R.S. 11:1006 are subject to an earnings limit. To minimize reporting errors on RTW1006 earnings, please remember:
  • Only report earnings through LSERS Web form - Retirees Return-to-Work Salary Reporting under Earning Limit;
  • Do not withhold or report employees’ contributions;
  • Do not include RTW1006 earnings in the monthly Salary Contributions Reporting files;
  • Terminate any RTW1006 retirees who are no longer working through LSERS Web form - Termination;
  • Report actual earnings, even if zero, each month for those actively working.
Timely reporting RTW1006 earnings will assist with providing accurate RTW1006 earnings limit balance to members. Please check the LSERS Web report - Retirees Return-to-Work 1006 Earnings Exception Report monthly. The individual earnings limit information is located under LSERS Web > Personal Information > Member Status. Please assist your return to work retirees with the information or encourage them to register for 24/7 online account access.

Reminder - Timely Reporting Monthly Salary Contributions

The monthly Salary Contributions reporting to LSERS is due on the 15th of the following month. Timely reported data will enhance the accuracy of online estimates and the efficiency of application processing. When the salary contribution information is not up to date, any members interested in retiring, refunding, transferring service credit, or participating in DROP or IBRP are affected. LSERS continually strives to reach new heights for improving the quality of service provided to our members (your employees). Your efforts must be counted on to reach this goal.

If you have any questions on reporting or uploading your monthly Salary Contributions, please contact Kimberlyn Stewart at kstewart@lsers.net or 225.925.4411.

And, as always, we appreciate and welcome any and all comments, as this helps
us identify ways to make improvements for you and provide better service to our
members and employers. Feel free to provide feedback and suggestions using
the LSERS Satisfaction Survey on our "Contact Us" page on the LSERS website.
